Through a social determinants of health program for a large midwestern Blues plan, PopHealthCare has conducted over 12,000 surveys to find out what members say matters most about their health.
The top five social determinants of health are: nutrition, 29% of those surveyed not receiving adequate nutrition; mental health, with 21% reporting stress or depression; physical activity, 20% reported getting no physical activity; disability, 17% reporting some form of disability; and financial strain, with 16% reporting that they struggle to pay for their basic needs.
By understanding these factors, PopHealthCare can help connect members with community resources and create a clearer pathway to better health.
